WISEWOMAN Evaluation and Technical Assistance

Since its implementation in 1995, the Well-Integrated Screening and Evaluation for the Women Across the Nation (WISEWOMAN) program has provided low-income, underinsured, and uninsured women ages 40 to 64 with health services to support prevention, management, and treatment of cardiovascular disease. Funded by the Centers for Disease Control and Prevention (CDC), the program's services include screening, healthy lifestyle support programs, and linkages to community resources.

To respond to shifting population, systems, and community needs, the WISEWOMAN program has evolved under the most recent cooperative agreement. In addition to the original program components related to screening and cardiovascular health risk assessment, the program model increases emphasis on supporting clinical systems of care to improve access and leveraging existing resources in the community. Lifestyle interventions have been reframed to include lifestyle programs and health coaching sessions, and minimum data elements have been updated to capture information about risk reduction counseling and participant readiness to change.

Mathematica has teamed with General Dynamics Information Technology (GDIT) to:

• Provide the CDC and its grantees with technical assistance to support data and evaluation activities,
• Develop a web-based reporting system,
• Conduct a systematic, multi-state national evaluation of WISEWOMAN program activities to determine progress toward program goals and outcomes.

Under this subcontract, Mathematica will lead technical assistance and evaluation activities, and will provide input and support to GDIT on the development of a web-based data collection system.
